Community Inclusion

Services are provided outside of a participant’s home to support and assist participants in educational, enrichment or recreational activities as outlined in his/her Service Plan that are intended to enhance inclusion in the community. Community Inclusion Services are delivered in a group setting not to exceed six (6) individuals. Examples:

• Small group recreational activities (i.e. events, shopping, bowling)
• Small group outings to community festivals, museums, books clubs, theater groups, cultural events, holiday celebrations, sporting events, etc.

Jesdit infusion LLC utilizes the design Individual Service Plan (ISP) assist the individual in reaching the desired outcome(s).
