Supports Program Services
The support program is a Medicaid waiver program that provides certain services for eligible adults with intellectual and developmental disabilities, age 21 and older, living with their families or in other non-licensed settings.
The program:
- Is designed to assist them to live in their communities.
- Provides opportunities for them to make their own choices and direct their own services.
- Provides all enrollees with employment/day services and individual/family support services; individuals and their families are able to choose from a variety of services, based on the individual’s assessed needs.
- Enables individuals who need both private duty nursing (PDN) services and supports program services to enroll in support program plus private duty nursing (sp+PDN).

Behavior Supports
Jesdit infusion LLC offers Individual and/or group counseling, behavioral interventions, diagnostic evaluations or consultations related to the individual’s developmental disability and necessary for the individual to acquire or maintain appropriate interactions with others. Behavioral management includes a complete assessment of the challenging behavior(s), development of a structured behavioral modification plan, implementation of the plan, ongoing training and supervision of caregivers and behavioral aides, and periodic reassessment of the plan.
Career Planning
Jesdit infusion LLC provides a person-centered, comprehensive employment planning and support service that provides assistance for program participants to obtain, maintain or advance in competitive employment or self-employment.
It is a focused, time-limited service engaging a participant in identifying a career direction and developing a plan for achieving competitive, integrated employment at or above the state’s minimum wage.
Community Based Supports
Jesdit infusion LLC provides direct support and assistance for participants, with or without the caregiver present, in or out of the participant’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan.
Community- Based Supports services are delivered one-on-one by our experienced DSPs to provide a high quality care to participants and may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living. Jesdit infusion LLC develops strategies to assist the individual in reaching the desired outcome(s).
Community Inclusion
Services are provided outside of a participant’s home to support and assist participants in educational, enrichment or recreational activities as outlined in his/her Service Plan that are intended to enhance inclusion in the community. Community Inclusion Services are delivered in a group setting not to exceed six (6) individuals. Examples:
- Small group recreational activities (i.e. events, shopping, bowling)
- Small group outings to community festivals, museums, books clubs, theater groups, cultural events, holiday celebrations, sporting events, etc.
Jesdit infusion LLC utilizes the design Individual Service Plan (ISP) assist the individual in reaching the desired outcome(s).
Individual Support
“Individual supports,” sometimes referred to as “personal assistance services,” provide an individual with the assistance he or she needs to live in, and become part of, the community. These generally are tasks and activities related to selfcare, personal maintenance, habilitation and household duties. Jesdit infusion LLC staffoffers this service in an individual’s own or family home, or in another community-based setting (such as a group home or a supported apartment) at the direction of the individual receiving the assistance, his or her family member or guardian.
Some examples of Individual Supports/ Personal Assistance Services include:
- Attendant Care Services
- Bathing in bed, in the tub, or shower
- Assisting in the use of a toilet or bedpan
- Grooming: hair care (including shampooing), shaving and ordinary nail cares
- Helping individuals transfer from bed to chair or wheelchair, or in and out of a tub or shower
- Caring for the teeth and mouth
- Changing bed linens with an individual in bed
- Assisting with eating and preparing meals
- including special therapeutic diets
- Dressing; o Ambulation (walking), indoors and out
- Escorting individuals to clinics, physician’s offices, related medical therapies, recreation activities and/or other trips
- Assisting with medication that can be self-administered
- Assisting with the use of special equipment such as a walker, braces, crutches, wheelchair, etc., after a thorough demonstration by a registered professional nurse or physical therapist, until that person is satisfied that the individual can use the equipment safely
- Assisting in implementing physical or occupational therapy, speech language pathology programs or psychological/behavioral programs.
Household Chores:
- Kitchen care, to include washing the dishes and making sure the refrigerator, stove, sink since and floor are clean.
- Bathroom care, to include cleaning the toilet, tub, shower, and floor; o Laundry, to include caring for an individual’s personal items and bed linen (may include necessary ironing and mending)
- Making and changing beds, washing windows, cutting lawns and putting out the garbage
Other household chores necessary for independent living:
- Routine errands, such as picking up medication or preparing meals
- shopping or any short trip to perform a specific task.
It is important to note that these services are not meant to isolate the individual from the community; therefore, the individual should accompany the Individual Support Staff wherever possible.
Interpreter Services
Jesdit infusion LLC providesinterpreter service to a participant face-to-face to support them in integrating more fully with community-based activities or employment. Interpreter services may be delivered in a participant’s home or in a community setting. For language interpretation, our interpreter service is delivered by an individual proficient in reading and speaking in the language in which the participant speaks.
Examples:
- Attendance at Service Plan meetings
- Communication with SCs
- Communication with providers
- Communications with FI
This Interpreter services may be used when the State Plan service for language line interpretation is not available or not feasible or when natural interpretive supports are not available.
Natural Support Training
Training and counseling services for individuals (any person, family member, neighbor, friend, companion, or co-worker who provides uncompensated care, training, guidance, companionship or support to a participant.) who provide unpaid support, training, companionship or supervision to participants. Training includes instruction about treatment regimens and other services included in the Service Plan, use of equipment specified in the Service Plan, and includes updates as necessary to safely maintain the participant at home. Counseling is aimed at assisting the unpaid caregiver in meeting the needs of the participant. All training for individuals who provide unpaid support to the participant must be included in the participant’s Service Plan. Natural Supports Training may be delivered to one individual or may be shared with one other individual.

- Training on use of AT device
- Training on a hover lift
- Training on ambulation/transfer techniques
- Training on dietary/eating techniques
- Training on diabetes management
- Training on implementation of behavior plan
- Training on PT or OT activities at home
Occupational Therapy
This service directs the individual’s participation in selected activities to restore, maintain or improve functional skills.objectives includeservice that address needs in the following areas: Fine motor; Sensorimotor including sensory processing/sensory integration; Feeding; Reflexes/muscle tone and other neurodevelopmental functions; Functional living skills including socio-emotional developmental needs; and Equipment including training, adaptation and/or modification.
Physical Therapy
Our Physical therapists provide services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities inpatients with injury or disease. Our physical therapists use creative methods to diminish pain and reeducate the body’s proper function by customizing a rehabilitation program around the specific needs of each patient. This includes evaluations, treatment, and education for individuals.Our experienced team will with work with your schedule to restore, maintain, and promote overall fitness and health.
Respite
“Respite” is care individuals receive when their parent(s), guardian or other immediate caregiver is temporarily absent or disabled.
Jesdit infusion provide respite service to participants unable to care for themthat are furnished on a short-term basis because of the absence or need for relief of those persons who normally provide care for the participant. Respite may be delivered in multiple periods of duration such as partial hour, hourly, daily without overnight, or daily with overnight. Respite may be provided in the participant’s home, a DHS licensed group home, or another community-based setting approved by DHS. Some settings, such as a hotel, may be approved by the State for use when options using other settings have been exhausted.
Transportation
Jesdit infusion LLC offers transportation serviceto enable participants to gain access to services, activities and resources. This service is offered in addition to medical transportation. “Transportation Services” refers to transportation that allows individuals to travel to waiver services and supports when it is not otherwise provided.
Examples of transportation services covered by the waiver include the cost of personal drivers*, taxi fares and train and bus tickets. The waiver also covers transportation services such as Access Link that are provided by municipalities, counties, or private contractors.
